Forest Dale Cemetery was first incorporated on September 7, 1901, to benefit and provide comfort for the families
of those buried on its property as well as providing a place of peace and serenity for those who are laid to rest here.
Our mission is to continue to provide a dignified and respectful environment where families and friends can visit
to honor the memory of a loved one.

Located in Crown Point near the intersection of Porter Mill Rd and Creek Rd just west of Veterans Memorial Park.
